Medical Requirements PDF Print E-mail

The University of Oregon requires documentation of immunity to measles (Rubeola) and mumps for all entering international students. Without this information you will NOT be able to register for your first term of classes.

WE URGE ALL ENTERING STUDENTS TO OBTAIN NEEDED VACCINES BEFORE COMING TO CAMPUS.

Please contact your local health department or your physician's office for any immunizations and documentation you will need.

Immunity to measles and mumps can be verified by getting signed statements from your health care provider or public health department on the Immunization/Exemption Certificate, or by presenting a copy of your school certificate of immunization. Day, month and year of immunizations must be noted. Notes from parents are not acceptable.

Exemptions may be provided for religious, medical or personal reasons. A signed Immunization/Exemption Certificate must be filed with the UO Health Center at the time of initial registration. Students with exemptions may be excluded from attending classes or other UO activities in the event of a measles or mumps outbreak on campus.
